Update to AJAX form to include drop down

I have a an AJAX form page that I need to be updated.
The current form submits data to an email address and then shows two downloads on the right after submission.
I need to add a dropdown select box to the form for Country. There will be 5 items in the dropdown (Ghana, Hong Kong, Japan, Nigeria, Singapore) with Please select: as the pre-selected item.
