Simple php work

hello there.

i need a simple php script that will take an uploaded image and user input text (through simple html form) and display the image in 2 or 3 different sizes along with the text as watermark. box shadow effect required only for 1 image.
the images must be available for 'save as' as being displayed in the page.
use of gd library preferred. need this done asap.
