Simple PHP / MySQL Database Search Page

Description of requirements/features: This is a simple job for a good PHP / MysQL developer who knows what they are doing.

I want to create a simple php script that searches a database based on some parameters I give it and then show me the total count and also at another push of a button see the actual dataset that the search has produced.

So I need a simple 1 page where I select from 5 different drop downs the 'tags' (seen in the attached database) that I want to filter the data with (note: could be more than one, but no more than 5). All the 'tags' that are in the database must be selectable in each of the drop downs.

Once i have selected the 'tags' in the drop down that i want to apply i then hit show result button and it shows the count of the number of contacts from the database that have those tags applied.

Below that I would then like to be able to push another button and then see the actual 'contacts' that have those tags applied.

I would also like the ability to update the database by having a box where I select a new .csv file to upload (in the same format as the one attached) which will then overwrite the old data and replace it so I can perform a new search and it will show in the drop downs any new 'tags' that are in that database.
