General information for the website: Sells 3 Services Online Which Are Restricted By Area - Customised Checkout Process With County Selection Via Map

Description of requirements/features: We require an experienced AJAX/PHP developer to develop a custom AJAX form for a website. This can be developed as raw HTML/PHP - or as a dedicated Joomla 3 module/component.
The form will have 4 steps;

Step 1: Select a product (1 of 3 radio buttons) - this passes product name and price to step 2
Step 2: Select a county from a HTML5 UK map (like: http://goo.gl/xJzKO8) - each county on the map will have 2 variables 'name' and 'service type'. 'Service type' will be 1 of 2 options. When a county is selected it will send the 4 variables (product name, price, county name and service type) to step 3.
Step 3: Shows a short message based on the 'county type' (1 of 2 messages - tells the buyer what level of service is available in their county). Also shows a form to capture name, telephone and email as well as the data captured in steps 1 and 2. Form validation required to ensure all fields are completed. When validated 2 'Checkout' buttons are available for PayPal and WorldPay.
Step 4: Pass the price and a unique order ID to PayPal/Worldpay checkouts (via a compatible method) and send the form data (including the county selected and the product/price) in an email to the site admin.
A checkout confirmation page will also be required once the Worldpay or PayPal checkout process is completed - this will show the order number and generate an automated email to the customer, which will explain that a member of staff will call them to arrange the service delivery date.
The form will ultimately be used within a responsive Joomla site.
The developer may use a commercial Joomla extension if one is available. This is a Joomla 3 site.
