Shoutcast / IceCast Radio Player Generator

Hi! I am needing to create a radio player generator that would play Shoutcast and Icecast stream links. We have a general idea of how we'd like the form to be, we'd like it to be an html so we can upload it to our site, it'd be a basic form where one enters the stream link and some customizable options like colors player designs etc, we have demos we can also send if this is something you think you can do please reach out to us.
