I would like a small admin system created so we can make this form Content Managed. So we would like to add the packages (separated by day time actives and night time activities (see example below) with the drop down feature with picture (see detailed in the example below). We would like a calender for the date to replace the existing combo boxes.
We would like the form to have the same functions as this form: http://www.henheaven.co.uk/oxford-quote-step1
