PHP Page to check file URLs to see if file is online

I have 400+ files duplicated over two servers, these are indexed by two MySQL servers in a Master>Slave replication configuration.
I would like a simple PHP page that allows me at a glance to see if any files referenced in the database are not live on my servers. I have the page framework code (all the HTML design to integrate into my system) which i have uploaded below and have commented the PHP space your code should go into.
The page should pull the list of files from the MySQL database, taking care that the "URL" field on the database contains 2 or more URL's, separated by a comma.
It should display all this nicely in a table with a simple check-mark if the file is online and a X mark if it is not.
The page should always try to connect to the master SQL server, however if connection fails it should fail over to the secondary server instead.
I have uploaded a example of what the output should look like, along with screenshots from MySQL so you can see the data structure.
