PHP Contact form HTML with Antispam Capability

Hello,

I need a contact form for my website with the built-in antispam function. All I need is for someone to generate the html code for me and I will insert it into the page myself. I have used iWeb (Mac) to design my site.

The form will be on an all-black background so please bear that in mind when choosing colors. What I currently have is attached. I want the same thing - the same colors, the same fields - but with a spam block function coded in.

This is an emergency project so when you bid, please note that I need a turnaround time of under 24hrs.
