PHP 7.4 Contact Forms / Mailchimp Integration

This job requires you to modify a PHP form script you own or to create one from scratch.

You also need to understand the requirements to submit to Mailchimp using the same PHP script. As we have been let down please reply with details of what is required (this will show us if you have the necessary experience). Replies without these details may be ignored.

Looking for someone to create 2 contact forms in PHP 7.4 (both need to be styled so that they can be pasted into a HTML based landing page).

Form 1 will need to be linked with Mailchimp so that a welcome email triggers

Form 2 is a simple post to email form with 7 fields and a drop down (4 selections).

Full layout and graphic for submit button available (correctly sliced) - so this is purely about creating reliable form scripts in PHP 7.4. We estimate this to be approx. 1 hour of work.
