Need additional fields and a document repository added to Sugar CRM (Opensource)

We have installed SugarCRM on our server which is an Opensource CRM. (http://www.sugarcrm.com/uk)

Within the "Sales" tab, there is a section called "Leads". This is what the screen currently looks like: http://awesomescreenshot.com/0072wzdg9e.

We need several additional fields added to this page. I have attached a second document (Assessing suitability Companies - PPH) which shows the fields to be added - they come under the master header of Client Information and then under four main headers: Company details; Directors; Shareholders and Beneficial Owners. All of these boxes under each of these headings need to be added along with any relevant text on the document.

In addition to this, at the bottom of the page needs to be a section where people can upload any documents relevant to that client, so a document repository. This repository will need to be able to hold an unlimited number of documents in all of the following formats - jpeg, Word and Pdf.
