Need a form coded wizard style (jquery + php) to add to DB

I have a registration form which I need to get coded up for a website, it has a few steps which I need to get written into the database from a user-friendly form with client-side validation.

Approximately 30 fields across 5 panels (but there is one option for locations + districts which needs to be able to track multiple districts which needs to be concatenated). The database is set up to work from a desktop app and populated approx 4 tables as well.
