Modifications & Enhancements for a big software project

We're looking for a junior software developer who has experience working in:
> PHP, Javascript, SQL, and CodeIgniter, or;
> SQL and C#, or;
> All of the above.

You'll be paid an hourly rate. You'll be given tasks that suit you the most by a software project manager (based on your strengths).

Please, provide a solution to a sample problem in C# (Windows GUI) or CodeIgniter:

Sample problem:

- Welcome Screen (#1):
- Text input for entering a personal number and;
- Radio button for choosing between 2 options: "one-time" or "continuous"
- "Next" button
- Screen #2 is displayed after "Next" button of Screen #1 is clicked:
- if "one-time" was chosen on Screen #1, a text "thanks for information" is displayed;
- if "continuous" was chosen on Screen #1, more data will have to be provided:
- Number input: "Asset volume";
- Number input: "Investment volume";
- Text input: "List of instruments"
- "Finish" button
- Screen #3 is displayed if "Finish" button of Screen #2 is clicked:
- a text "thanks for information" is displayed
- Save all entered information in a SQLite database.
