Minimal Form with CSS Animations to be created

Description of requirements/functionality: We are looking for a four stage form to be coded.

The form will ask for how much they owe, name, email and phone number. We currently have a four stage form on our website www.freshfinance.net which is powered by Form Keep. We are after our own version to be coded without having to use Form Keep and that also utilises a three choice button selection on the 1st stage of the form.

The form we want to build is based upon the Minimal Form example by Codrops here - http://tympanus.net/codrops/2014/04/01/minimal-form-interface/. We have created a styled version but hit a wall with our development skills.

There are a few issues with this demo. Firstly we want to replace the first question with a three choice button selection.

Secondly this demo has poor validation, and we would need this being rewritten.

Thirdly we want to incorporate subtle animations throughout this form, which will vary from desktop & tablet version to the mobile version. These are referenced in the attached pdf.

Note: Due to the nature of animations being a moving object and our best attempts to describe them being static, we expect that changes/tweaks will need to be made before the development process has concluded.

The form should send two emails out. One to the customer in Multipart MIME HTML and plain text formats. The second to ourselves in plain text format with the form data included.

The form like the website should be responsive and work across all browser widths correctly.
