JQuery/MySQL/PHP/Ajax based Questionnaire (Multipage form)

I need a jQueryAjax/PHP/MySQL based questionnaire on Thickbox. See the attached screen.

1. Multi-page Questions are getting displayed from calling a routine in mySQL
2. Score is captured in another table from out of 7 radio button levels
3. Need pagination on the Grid/form
5. After 'Next/Submit', If the questions left, a red box will get displayed around the question.
6. While moving to the Grid/Form back and forth, the responses of the user should not get lost.
7. Before 'Submit', user can change any responses.
8. Fast loading of the questionnaire grid/form. Must use Ajax.
9. Need flexibility(functions) to change the number of rows per page, column hide/un-hide, column name change, sorting in the script.

I will provide transaction table to store responses, a routine to get the questions for the grid/form.
The output of the work must not less than expected as per screen.
