Description of requirements/features: Adapt an existing "lightbox" web form (triggered via live chat) for two different use cases. Each requires the data from the form and the live chat transcript to be sent to a specified email address upon completion of the form and chat. Both also require CSS design to fit the respective brands. You can view an example of an existing use with the attached video.

Version 1:
Preferred date and time selection options

Version 2:
Postcode entry
Lookup closest location from database
User adds preferred date and time as with Version 1
Sent to an email address which matches the closest location
Extra notes: We're looking for a freelancer to join our team long term for ongoing development work. The successful candidate from this initial job will be offered to become a part of the team and our "go to guy/gal" for all future work which matches their abilities.

Moving forwards I'd like this candidate to be able to work with us to develop and implement new idea's alongside the rest of the team.
