Issues -- personal php/mysql application

I have a small personal php/mysql application.
That is used to capture user data. from a questionnaire.
It works.. but i am having some problems with the logic, when i hit an upper boundary,
I'm not familiar enough with php / mysql to use best practice to fix it. I'm pretty sure a php/mysql person could quickly solve it.
A user has to read some text then answer questions 1 2 and 3 (where each question has three possible versions A,B and C), so a users must respond to question 1,2 and 3, and also to have used a version A + B + C, it is just the logic to do this i am strugglinging with.
