Integrate an upload feature to an existing google form

I have an existing google form to which I would like to add an "upload" feature.
knowledge:
google apps, google scripts.

Requirements:
1. A student fills a regular google form which should be available to view by any one with the link
2. on the form there should be a place for the user to attach multiple files by browser and /or Drag and Drop (something similar to the upload feature that can be found here on PPH forms).
So, the user adds the files and it should be uploaded to a specific folder on google drive.
while upload takes place, some feedback about the progress of the upload should be presented to the user.
3. by the end of the upload process the user shall be able to submit the form.
4. the user should be notified if all data submitted as it should or receive failure message in case of error for some reason'
5. All the collected data shall be listed on a google sheet.
6. the sheet must include URL to the uploaded files. if the user uploaded few files all links shall be listed in the same cell while each URL is in a new line.
7. email notification shall be send to and shall include the submitted data.

the job as to be delivered as follows:
1. developer shall integrate the scripts and code into the form.
2. developer should add small guide explaining how to copy this features to other forms in case of need.
3. work should be delivered no longer than two weeks after project acceptance.
