I am trying to design a fillable word document/form/template or outlook form to increase referral from hospitals to clinics of patients in South Africa.
This form will be completed by emergency doctors who have literally no time - so it has to auto populate and be super slick and quick to use. I don't know if this is in your sphere of work, but I'd be very grateful for your help and of course very happy to pay for your time.
