I need a web page designing that will populate a certificate with data collected

I have a piece of quiz software the produces a report at the end with the results of the quiz. Instead of it being in the format of the simple table, I want to take three or four piece of information and create a certificate, using a jpeg certificate I have already prepared as the template.

I've attached the html file that shows the code of the current report page, and a print-off of the format the report currently takes. I've also attached a jpeg of the certificate template - you'll see that I need it to populate with the name of the student, the date completed, the test score and the result (Passed/Failed).

I would like a revised report.html file back that incorporates the graphic template, and will include the students details using the relevant field codes that have already been collected.
