I need a web developer to add a jQuery Modal / Pop-up Confirm box to single web page

Description of requirements/features: I need a web developer to urgently add a jQuery Modal / Pop-up Confirm box to single web page which asks the users if they wish to continue leaving the site.

- There is only one page on the existing website - the index.html page
- I have added a link in the top right called 'US Corporate Site'
- When a user clicks on this 'US Corporate Site link, I'm looking for the pop- to appear with two lines of text that I can style with css, and a 'yes' and 'no' button.
- The no button closes the pop-up to return to the index.html page
- The yes button links to another site in the same browser tab
- I have a jpeg of who I'd like the pop-up to be styles with the correct text on it

When the pop-up appears, the whole site should grey out with a transparency whilst the pop-up is overlaid on the top like this eaxmple: http://bootboxjs.com/
