I need a simple program built for the PC

I need a simple program written for the PC. It would allow employers to enter info on their employees and their pay for a certain time period and then get a printout that informs the employee of what their total employee compensation package is.

All the details are in the attached document including a sample of what the final printout would look like. This is really just a simple program that asks questions and then provides a field for the answer to be entered. For example, it will ask how many hours an employee worked during a time period and then it will ask what his Workman's Comp tax rate is. Then, to get the total of that tax cost for the time period, it will multiply the tax rate times the hours worked (or in some cases, times the gross pay).

It will also allow for the employer to do it on an Excel spread sheet and then upload all the data into the program.

And, finally to print it out on one page per employee for distribution.

I think I would prefer a US programmer because you will understand the tax terminology better and may even have a few ideas of your own to contribute (what would you like to see on a report for yourself??)

I have built this program myself before in BASIC but those days are gone. However, that means I know how easy this is. I just cannot program in C or whatever language works best to get this done and make it look attractive. If you have questions after reading the document attached, let me know. I would like to get this done relatively quickly. If you are good, I have more I want to do.
