I need a php voting script made.

General information for the website: Free Product Giveaway
Description of requirements/features: I need a special voting script done for one of my sites. I will show you an example that I have on one of my sites. I need the exact script put on a different site of mine. The guy that designed the example script only took like an hour to do and charged me $20. So I am hoping it is a rather easy script to write.
What it does is supply a form that people have to fill out EACH time they vote and when they submit it. It writes it to a text file. The script also puts a cookie in their computer so that they can only vote once every 12 hours. It also compares IP addresses. The example script uses a "photo_ID" thing to tell me who they voted for. So I need the same type of ID so that I know WHO and what product they voted for.
Here is an example of how it writes to the text file:

Keep in mind that the one on the Product Giveaway site does NOT work. I need it replaced with a new script. I also need the script to work even if I put product pages in different directories.
I do not have a large budget, So I will go with the lowest bid that can do the job correctly at a low rate.
