I need a form on salesforce to be coded and add features

Hi i need an existing from template coded with below points

A smaller form, we’re currently transitioning to a pop out form design but the form currently takes up the entire screen (example below). We need a smaller form.

Currently the form boxes are too large, as well as the entire form being smaller, we also need smaller boxes.

We need the form box headers such as name/ job role to be inside the box not outside.

We need at add functionality for the country field that will bring up country suggestions as you type.
