I need a "dummy copy" of a public online application form

General information for the business: For students to practice with
Description of every module: Applications for funding to the Arts Council England grants for the arts programme is now done through an interactive online programme. You can find it here https://applications.artscouncil.org.uk/frontOffice.jsf. I need someone who can create a cheap and cheerful "dummy" version of the online application form so that students can practice filling it in to the exact specifications required and then download their "dummy application" as a pdf.

I don't especially need the students to create a profile. I do need them to be able to register as an individual or an organisation and fill in all the questions. The easiest way to find out will be to go onto the arts council's website and register and find out.
Description of requirements/functionality: Ideally students should be able to be given a link and fill in the form and download the pdf for marking
