Description of requirements/features: Application form:
Long form comprised of 3 fieldsets on the same page. Very basic styling of fields - this will be plugged into an existing design afterwards:
1) 11 regular fields
2) 9 regular, 2 x 3 field rows e.g. field1, field2, field3 ("Add more" functionality required), 1 x 2 field rows e.g. field1, field2 ("Add more" functionality required)
3) 9 regular fields
Data must be added to the MySQL database on submission. Fields must be sticky (e.g. not losing value after server side submission - stored in session and displayed and escaped). Form should be secure (no XSS/SQL injections please).
Server and client side validation to check mandatory fields present. Client side validation only necessary for date fields (jQuery datepicker will do), and one field which has a requirement of being no longer than 150 words.
As it's a long form, I'll need an XHR call in the background to run every min or so, just to keep the session alive.
Application cron:
A PHP script executed as a cron needs to be able to run through the database, check for recent submissions and send out an email to applicant and admin to say the application has been received. The template doesn't need to include personal fields.
CMS and Admin requirements: A simple view of all applications, and a page to view each application in detail (all fields displayed). No design needed, just stick a CSS framework like Bootstrap on it and that will be fine. Although being able to print the applications is a requirement - if that influences your approach.
Should be able to work in IE8.
No user account functionality needed - will be setup behind HTTP auth.
Extra notes: This must be developed in PHP/JS/MySQL. If a PHP framework is used, it must be compatible with a shared hosting setup on Hostgator. Must be PHP 5.3 compatible. I expect you to be comfortable with OOP. I'd rather not have a big procedural mess, as I'm the one who will be maintaining it afterwards!
The database design should be normalised (e.g. the rows where multiple values can be added - they're ideal for a separate table), and I will require the schema (SQL dump of the structure).
FYI I'm a developer by trade, it just so happens I have a surplus of work at the moment. I'll be at hand for any technical questions throughout the project. If this one goes well, hopefully I'll have my go-to-person for future outsourcing :)
