Fix PHP errors, Wordpress plugin errors

Errors :

1. Ordering a card and creating a signature is supposed to create a jpg file and write to disk - this is not happening for all cards, suggests a permission error.

2. Order codes are generated as a concatenation of the letter F and a six digit number - some orders do not have this

3. DB-toolkit plugin in the backend has some errors, it works fine but there is a modal window which cannot be closed once it pops up, again I think this is a permissions error.

4. Autoincrement order codes need to be changed to start from a higher range.

5. There are some instances of shortcodes being exposed on the site, need to remove them.

Finally all cards need to be tested to make sure your changes have been implemented correctly.
