General information for the website: Selling service agreements for boilers
Kind of development: Customization of existing website
Description of requirements/features: This is a simple job. As attached in the pictures I have updated the contact form on index.html by adding 3 new boxes, Address, Postcode and Phone. I need the person to edit the existing JS and PHP script to make the contact form work with the new boxes.
I have also attached the PHP script so that you are able to see how it was before. When accepted, I will send you the files for the website.
