Create Excel Spreadsheet

1. To create a one page excel spreadsheet with approximately 21 columns.
2. To apply values to most of the columns so they can be totalled. For example, gender (male/female); ages (11-15; 16-24; 25-34; etc.).
3. To set up formulas for the columns so they can be added up.
4. To create graph to illustrate numerical changes between two columns
5. No data input required, i.e. the spreadsheet will be blank. I would just like someone to set it up.
6. Would be good service if the provider i accept could provide an email address for me to contact at a later time if adjustments need to be added.

Example of how it should look is attached but there are no functions/formulas.
