Create a script to create HTML Files from a form

Description of requirements/features: I am looking for a script that will allow is to create html files using the code attached.

A webpage should be created with 1 form field on it asking for a url and a submit button. When the submit button is pressed a html file is created on the server using the code below and where it says "FormDataGoesHere" in the html file, the url that was submitted replaces it.

The files should be created in order for example ptm_file_1.html ptm_file_2.html and so forth.

The code each HTML file should contain is attached.
