Contact form generated by external JSON file

I need exactly the same contact form as on this page here: https://www.numbersupermarket.co.uk/buy-number/

Numbers should be generated from an EXTERNAL JSON file in the same or similar format:

The form should be sending an email with the selected package and number.

Ideally this contact form will be written in simple PHP, HTML, JSON and maybe some JavaScript.

You don't need entering all numbers in the JSON file. Enter just a few so its enough to make the functionality work. I will update the JSON file with all the numbers later.

Don't worry about the styling, I mainly need functionality.
