Contact form generated by external JSON file

I need exactly the same contact form as on this page here: https://www.numbersupermarket.co.uk/buy-number/
Numbers should be generated from an EXTERNAL JSON file in the same or similar format:

{"0330":["0330 332 6178", "0330 332 5489", "0330 332 5702"]},
{"0800":["0800 074 8615", "0800 056 0720", "0800 056 0720"]},
{"0113":["0113 345 2250", "0113 345 2241"] }
]
The form should be sending an email with the selected package and number.
Ideally this contact form will be written in simple PHP, HTML, JSON and maybe some JavaScript.
You don't need entering all numbers in the JSON file. Enter just a few so its enough to make the functionality work. I will update the JSON file with all the numbers later.
Don't worry about the styling, I mainly need functionality.
