Complete a HTML Responsive Website & convert site to PHP/simple CMS

General information for the website: It's a doctors website utilising a template that's already been installed. Need to complete the site and also convert it to PHP/CMS

Description of requirements/features: This project is in 2 parts:

Part 1 - immediate deadline
Website is already there (www.drkevinmoore.com) - a responsive website template was used. Images and copy will be provided and all needs to be uploaded to site. The contact form needs to be created and a new vertical side menu created for all pages except the home page. Also the design probably needs a bit of an facelift - it's a bit plain. This needs to be completed as soon as possible.

Part 2 - less urgent
I want to convert the html pages to PHP and develop a simple CMS system and end up with a website where I can reuse the website again and again - I want to use external includes - config files which store customer variables that I can update as required. CSS must be separated out completely from the code so I can tweak it and get a completely different look by changing colours/text etc.
A simple menu management system is also required. A simple logon system to a dashboard.php file where a user can edit page content and an admin can do additional tasks. There will be some extra features such as an internal email system and SMS system to be implemented - code will be supplied for these. I also want to be able to implement a hook up a simple payment screen via PayPal.

Sample code from other projects will be provided if available otherwise it will need to be developed from scratch.

Further full explanations will be provided to the chosen programmer. I will be available on Skype Chat (chat only - no access to speakers/microphone) to oversee project and answer questions. Very good written English is a MUST. I simply don't have the time to be explaining things again and again.
