C# - very small utility for Windows 2003/2008/2008R2/7 - match partitions to physical disk

I need someone to create a small utility
for Windows -simple GUI with a button
and output in the same window + check box for a single
option:

The utility must list all partitions on the current computer
(Windows 2003/2008/2008R2/7) including the mount points
and identify the underlying physical disk (by real
identifiable characteristics such as real serial number).

This of it this way: in a server with 16 identical sized disks,
I need to know what real disks are forming a partition.

The check box will allow during the check to write a very small
file to the disk, which can then be used to make absolutely
sure that the physical disk is the one identified.

Upon delivery all code will become my property and all rights
will be transferred to me.

You can work on this for 2 weeks or so if you want
I would need this preferably in C# and I need the exe/msi
and all code.

Please bid with your best offer, more utilities are on
the horizon.
