Awazchat registration and admin control panel

Project Description:
We want to build registration form ,admin control panel , user datasheet and email domain tool.

Registration form:
We want to build simple registration form for our Awazchat Application name,email,userid ,password
Email verification:user registration will not be completed unless user verify his email by clicking link.
use of valid email domain: only valid email domains are allowed like yahoo.com,hotmail.com,gmail.com form validation can be used to validate domain
with javascript.
ip loging : ip of member must be added in user database to stop multiple registration from same ip at the time of registration

mac address:mac address of user can be also be added in user database at the time of registration

Ban functions:
1.Ban email ,Ban IP ,BAN MAC address options should be available to ban any user for this user ip ,email and mac address should me stored in userdatabase at the time of registration.
2.Make admin option should be available to make any user admin to access admin control panel
3.Edit or delete option should be available for userdatabase modification.

Admin control panel:
Simple Login window for admin control panel once inside admin can select mentioned options

1.Enable disable to hide main member registration form in case of spammer attack.
2.Enable disable to hide main member login form in case of spammer attack.
3.Add email domain utility to add more authentic domain to avoid spamming email or fake email address.
4.userdatabase enter userdatabase to modify,ban,unban,delete or make any user admin.

Email domain list
1.Add /Delete authorized email domain.

please see attached excel sheet for story boarding ,its very simple form based tool due to its simplicity please quote reasonable bid for this project
