Adobe Dynamic Form

I require someone to create a dynamic/ form based pdf of the attached form. I have provide 2 examples (one in .wrd and another in .xls). I would like to bring both these documents to create one single pdf form.

Pdf form should be editable by user - they should be able to add/delete lines.
Pdf form should automatically calculate review date based on the date completed by user
Pdf should multiply and colour code risk ratings
