Windows tabbed form to a set design with instructions

Hi

Im looking for a windows form with tabs to a specific style (I have attached a screenshot for look and feel). The form should be 640x480 with 5 tabs and a few fields within the forms. I would also like the documentation on how you completed this so the job could be replicated this could be in the form of written instructions or a video of your completing the work from scratch. In your bid please let me know what tools you will be using i.e. silverlight, visual studio etc.
