PHP/MySQL Web App needed in double-quick time ! 5 stage job - this is stage 1.

So - Thanks for looking - this is an urgent requirement !

I have to develop a new web-app for a potential huge client for my business, and this is stage 1 of the development so I can go and demonstrate look and feel to them. There are multiple stages to this development, each one a separate job that will be paid as such.

The industry sector is vehicle management, my business manages fleets of vehicles for customers, and the information now needs to be web based and at my customers fingertips.

I want the web app to ideally be PHP based, hosted probably with names.co.uk or similar and use a mysql db.

The web app will involve tiered user access, various permissions and some forms to be filled in (please see attached info for more detail).

The long term goal is to make this into a business system with users from clients, and my own staff using to do different things. There will be a requirement for an android app to feed into the db, and also various web services to allow two-way communication between this new app and my existing database which is housed on site.

I've mocked up a dummy of the various pages I need the webapp to have initially along with a rough DB schema. The ideal candidate will look at this, understand what I need and be able to develop rapidly ! I have a meeting on Thursday 18th July where I want to demo this in a working environment. I am committed to this project 100% and will work with you as much as possible to get this up and running, I can set up hosting, DNS etc. to make it all happen - I need an expert developer to get this up and running for me. The information is attached below as an excel spreadsheet, each tab being a web page - but the ideal person will pick this up, understand it and know a far better way of doing this than me !

I have put a price based on what I think is right for this stage , the next 4 stages are negotiable on price depending on complexity and I am looking to build a long term relationship with the right software developer(s). I have loads more projects in the pipeline.
