Need an iPhone app to be done for all the resolutions

General information for the app: this app is used to maintain the details of the medical history of the user

Description of requirements/features: hi,

this app is used to maintain the medical history of the user, i am listing the some of the features, remaining all are present in the wireframe

1. login , signup , forgot password
2. Forgot password would use web-service
3. It has the hamberger menu
4. once user logged in , he would be adding all the family members and thir medical history
5. there we would be displaying all the saved records in table view
6. this app has to maintain local database
7. for ever medical record PDF needs to be generated once it is saved in the database
8. user would have an option to export / email the pdf to others
9. Search option is there. it will search all the area of the app, like user name ,medical record name ,medication details , pdf names
10. app has the remainders fecility so we need to set local notifications to alert the user
Specific technologies required: objective c , cocoa, Autolayout, swift
Extra notes: Initial version should be completed with in 5 days
