Iphone app, incident reporting

i rerquire an app for reporting incidents for my security company and for resale to other companies world wide. it needs to cover several SIA sectors including, alarm response, security guarding, executive protection and door supervision.

simple mulitple choice and text input boxes should populate a word template incident report that is emailed as an attacment or email body from the hand set

will take advice on other features such as location services, time stamps, photo uplioads, echosign intergrstion etc, but not essential at this time.

iphone first then android if we find traction.
