IPad Mini QR Code/Bar Code Orienteering style Question App

I need an app that will work on an iPad, and maybe an iPone and Android phone.

I run a Radio exercise where Army Cadets travel around a course to answer questions. I would like to give them a map of where a QR code or similar is. When they arrive at the location and find the QR code, they read it with the app and the app reveals a question they need to answer before they move onto the next clue.

I have a couple of old 2013 iPad Minis which it would be good to use. I can buy more (I need circa 6-8 in total for up to 8 teams), or they can just use their phones.

I will need to be able add the QR codes and Questions, and also be able to save multiple blocks of codes for different courses in different places, although I suppose it doesn't matter if they are all filed together. Some category option may be a good idea. I need to be able to brand it, or I can send the artwork to add them.

I have attached a very rough idea of what the phone screen version may look like.
