IOS/Android App Form

I'm needing an application developed in a form format that allows a member of staff to pick different images/items on a form that then creates an exportable pdf with all the chosen images on it.

there needs to be information on each image and different sections. These need to be easily customisable by us in house so we can populate the app and edit it when necessary.

The exportable PDf needs to be in a format thats easy to read and pleasing to the eye to send onto clients.
