File Move script

This should be easy for anyone interested. What i need is a script that that will search a volume for a specific file type, and move the files to a folder that the script creates based on the file creation date. Specifically mm/yyyy.

User inputs should be:
1. File type (i.e. jpg, doc etc.)
2. Volume to search
3. Destination volume and or subfolder. The script should create in this location a set of subfolders for each year and month and then place the files in the appropriate folders.
please get back to me if you need more information. I tried to do this with various tools but none of them support moving by CREATION date - only mod or access. I have also attached a vbscript I was working on but I am not a vbscript person and it is very difficult for me - and it doesn\'t work :)
