Excel for Mac 2004 VBA programmer

Convert Excel for Windows 2003 application to Mac with Office 2004. A simple app. All VBA code already well tested (not elegant but works).

User enters data on 3 simple tables:
Location (name, address, representative, role, phone, fax),
Prescribers (name, NPI#, license#, type)
MPS (name, email, phone, yes/no question#1, yes/no question #2)

Generate 2 assignment templates (to assign Prescribers to Locations; to assign MPS to Prescribers).

Produce 1 report.
Generate one contract per Location.

Convert remotely on my Mac using Teamviewer ot similar.
