***Create an Online Form andDatabase - PHP5***

Turn this form into an online app. Note it has 3 pages and drop down boxes, formulas etc. It also needs to look presentable (i.e. not scruffy!). Effectively it is to manage what sales people have earnt and are due with which clients, how much revenue they are bringing and what commission they are owed.

There are 2 admin levels, 1 for myself where I can see everyones accounts and what they have earnt the business in total.

The second is a sales admin level where they get to input and update client info and it displays how much commision they have earnt and are due for that month. They do not see how much they have earnt for the company in total and cannot see other sales user details.

The excel spreadsheet is a rough guide, happy to accept a simplified or better version, as long as all the key data is included.

I also need to be able to add and delete users.

The job will be tendered to the best price with the most experience and that can be easily contacted. Other work may well be given after this if done to a good standard.
