Browser workflow form for Oracle IPM (was Stellent) with action buttons

My client requires a form to be developed to present existing Oracle IPM workflow package data in a browser-based form, with two action buttons (Accept/Reject) plus user comments. Key task is for the action decision to be written to a bespoke log table, with date, user ID etc, the form to close and the package to be set as complete in the queue, so moving it on to the next process. Additional features to be considered:
- Validation of the IPM User ID against the Active Directory for the required user
- Single-step login from a URL to the package and form.
