App development 6 week summer project

I'm looking to offer some summer work to a budding App development wizard. No travel required but must be willing to put in the hours and produce a finished multi platform App from a specification. Essentially this will be a bit like survey monkey except it will be for an individual to collect answers to set questions from multiple product suppliers (the products will all be service desk software). The idea is that they can then compare all the answers/scores and use this information to help select a product/supplier. Nice UI/UX essential. Deadline for completion mid August. Please see the attached background document which include the type of questions that will be included.

I will need to see examples of what you have already done. Please don't send me a proposal without reading the attached document ;-) Further work may be available for a larger/more comprehensive version of this initial App.
