A tick box survey form that produces an end report

We carry out building surveys. Each survey has 8 headings with various data inputted to each. For example type of house is terraced, bungalow, semi detached, detached and was built in 1930, 1940, 1950 constructed from solid brick walls, cavity brick walls, rendered walls and so on.
I want to be able to simply tick the relevant box on my iPad or Microsoft tablet to a series of questions and dependent on answers end up with a completed survey in word format.
Edit - this is for my sole use only - I was thinking it was something that could be done in word if possible?
