Appointment schedule

Hi
I need a software which could keep my appointments.
Field name should be dynamic and the report should be generated based on the field name selected.
For example, will create field names like: Name of the person, meeting date, meeting agenda, Minutes of the meeting etc.
Once the fields are made, will fill in the required data.
Report generation to be made based on field selected. Will also need a search option.
