Sage Pay Form Integration for taking card payments

We use Sage Pay to take cars payments online. We had a company create a page on which the form was created to pass non confidential information over to Sage Pay for the client to then enter their card details and make payment to us and upon completion, the user is redirected back to our site - standard form integration through Sage Pay.

Well, the current page needs updating to work with the new server its hosted on. I have attached the error we are receiving on the page.
