Prestashop - resolve display issue on contact form page and other if possible

I have a website which was recently moved, however there is an ongoing issue that I cannot identify.

The attached images should illustrate the problem, these are:

1. The formatting of the contact form are corrupted.
2. The horizontal "contact" menu item is displaying on a second line (in Internet Explorer only).
3. There is a small gap between the horizontal menu items that I would like removed (optional).
