Mysql search

I have a site on which there is a form with a jump menu which populates its associate giving main and sub categories this form posts to a mysql database, I need a search script which uses a similar method to query the database, there are 21 fields in the database although if three of these fields could be searched Main Category Sub Category, Counties And the results printed out as below.

Company Name
Address 1
Address 2
Town
County
Tel
Fax
E Mail
Website

Description,,,,,,,,

You can view the database output and search form at oracleireland.com/admin, I will need other scripts when this is implemented.
